- Contract of Employment with English Translation
- Remittance slips (6 months) / Payslips (3 months) / Bank Statements
- Consularized Special Power of Attorney
- Details of crewing manager (for seafarers only)
Additional requirements for SELF-EMPLOYED
- Certificate of Business Registration / DTI
- 6 months working account /bank
- ITR with Financial Statement
- Three (3) Major List of Suppliers (Supplier's Name, Complete Address, Contact Number, Contact Person)
*All certificates must be up-to-date and issued by the Philippine Statistics Authority (PSA)
