• Remittance slips (6 months) / Payslips (3 months) / Bank Statements
• Consularized Special Power of Attorney
•  Details of crewing manager for seafarers

IF SELF-EMPLOYED

• Certificate of Business Registration / DTI
• 6 months working account /bank
• ITR with Financial Statement
• Three (3) Major List of Suppliers (Supplier's Name, Complete Address, Contact Number, Contact Person)
